APPROVE CONSOLIDATED CONTRACT #CLH31017 AMENDMENT 11 BETWEEN LEWIS COUNTY AND THE WASHINGTON STATE DEPARTMENT OF HEALTH (DOH)
WHEREAS, the Lewis County Board of County Commissioners (BOCC) has reviewed Consolidated Contract #CLH31017 Amendment 11 between Lewis County and Washington State Department of Health (DOH); and
WHEREAS, Amendment 11 increases funding by $19,750 for a new total maximum consideration of $5,186,045; and
WHEREAS, Amendment 11 will amend the Statements of Work for the following three (3) programs:
- DCHS - ELC COVID-19 Response
- Office of Drinking Water Group A Program
- Office of Immunization COVID-19 Vaccine
WHEREAS, it appears to be in the best public interest to approve Amendment 11 for the Consolidated Contract #CLH31017.
NOW THEREFORE BE IT RESOLVED that the BOCC hereby approves Amendment 11 to increase funding by $19,750 for a new total maximum consideration of $5,186,045 and amend the Statements of Work for the three (3) programs listed above and authorizes the Director of Public Health & Social Services to sign the same..
