APPROVE CONSOLIDATED CONTRACT #CLH31017 AMENDMENT 10 BETWEEN LEWIS COUNTY AND THE WASHINGTON STATE DEPARTMENT OF HEALTH (DOH)
WHEREAS, the Lewis County Board of County Commissioners (BOCC) has reviewed Consolidated Contract #CLH31017 Amendment 10 between Lewis County and Washington State Department of Health (DOH); and
WHEREAS, Amendment 10 increases funding by $499,203 for a new total maximum consideration of $5,166,295; and
WHEREAS, Amendment 10 will amend the Statements of Work for the following three (3) programs:
- Office of Drinking Water Group B Programs
- Office of Immunization COVID-19 Vaccine
- WIC Nutrition Program
WHEREAS, it appears to be in the best public interest to approve Amendment 10 for the Consolidated Contract #CLH31017.
NOW THEREFORE BE IT RESOLVED that the BOCC hereby approves Amendment 10 to increase funding by $499,203 for a new total maximum consideration of $5,166,295 and amend the Statements of Work for the three (3) programs listed above and authorizes the Director of Public Health & Social Services to sign the same.
