Summary:

The purpose of this amendment is to make additions to the statement of work and revisions to the budget detail to reflect changes in allowable activities and reimbursement as directed by the Developmental Disabilities Administration. These activities are related to Individual Supported Employment and Community Inclusion services for persons with developmental disabilities and include but are not limited to professional development, client support hours, and outreach.

Resolution

WHEREAS, the Lewis County Board of County Commissioners (BOCC) has reviewed Amendment #R01-0135-22-A to professional service contract #R01-0135-21 with Reliable Enterprises to provide services to individuals with developmental disabilities for Individual Employment, Community Inclusion, Child Development, and other supportive services; and

WHEREAS, the purpose of this amendment is to expand the statement of work and revise the budget detail to reflect changes in allowable activities and reimbursement as directed by the Developmental Disabilities Administration; and

WHEREAS, these activities are related to Individual Supported Employment and Community Inclusion services for persons with developmental disabilities and include, but are not limited to, professional development, client support hours and outreach; and

WHEREAS, the contract duration is July 1, 2021, through June 30, 2022; and

WHEREAS, it appears to be in the best public interest to authorize the execution of said contract.

NOW THEREFORE BE IT RESOLVED that Amendment #R01-0135-22-A to professional service contract #R01-0135-21 between Lewis County and Reliable Enterprises to make additions to the statement of work and revisions to the budget detail to reflect changes in allowable activities and reimbursement as directed by the Developmental Disabilities Administration from July 1, 2021, through June 30, 2022 is hereby approved, and the Director of Public Health & Social Services is authorized to sign the same.
