Resolution

WHEREAS, the Lewis County Board of County Commissioners (BOCC) approved Amendment B (Birdseyemedical_MassVacCOVID-19-B) with Bird’s Eye Medical via Resolution 21-285 on August 3, 2021; and

WHEREAS, Amendment B provided an additional $280,000 in funding, increasing the total amount of funding provided for the period of January 21, 2021, through October 18, 2021, to $1,030,000, but it contained a scrivener’s error that incorrectly identified the total amount of funding provided for the coverage period as $1,360,000 on the agreement; and

WHEREAS, Lewis County and Bird’s Eye Medical wish to correct the scrivener’s error through Amendment C (#Birdseyemedical_MassVacCOVID-19-C), which corrects the total amount of funding to $1,030,000; and

WHEREAS, services are added to the Statement of Work and Reporting Requirements; and

WHEREAS, it appears to be in the best public interest to authorize the execution of said contract.

NOW THEREFORE BE IT RESOLVED that Amendment C (#Birdseyemedical_MassVacCOVID-19-C) between Lewis County and Bird’s Eye Medical to provide COVID-19 vaccine consultation, coordination, and administration corrects a scrivener’s error providing a total of $1,030,000 from January 21, 2021, through October 18, 2021, and changes the Statement of Work is hereby approved and the Director of Public Health & Social Services is authorized to sign the same.
