Summary:

PHSS receives funding from the Washington State Department of Health to prepare, prevent and respond to the COVID-19 pandemic. The purpose of this amendment is to extend the MOU duration, add to the Scope of Work and add funds. Activities preformed through this MOU shall include COVID-19 testing and vaccine administration for persons residing in Lewis County.

Resolution

WHEREAS, the Board of County Commissioners (BOCC), Lewis County, Washington, has reviewed Memorandum of Understanding Amendment #Valley View Health Center-A between Lewis County and Valley View to provide COVID-19 testing and vaccine administration for Lewis County residents; and

WHEREAS, this amendment will be effective December 28, 2020, through June 30, 2021; and

WHEREAS, funding is provided from the State Department of Health (DOH) in the amount of $100,000 added to the original amount of $400,000 for a maximum total amount of $500,000; and

WHEREAS, it appears to be in the best public interest to authorize the execution of said contract for Lewis County.

NOW THEREFORE BE IT RESOLVED that Memorandum of Understanding Amendment #Valley View Health Center-A between Valley View Health Center and Lewis County in the amount of $100,000 and extending duration through June 30, 2021, is hereby approved and the Director of Public Health & Social Services is authorized to sign the same.
