Summary:

The Consolidated Contract is the primary vehicle for transferring federal and state funding to Local Health Jurisdictions (LHJs) for planning and local implementation of public health prevention and early intervention programs and activities.

The purpose of Amendment #2 is to (1) increase funding by $43,648 for a revised maximum consideration of $2,945,392, (2) add Statements of Work for one program, (3) amend Statements of Work for one program, and (4) amend Exhibit C Federal Grant Awards Index.

Resolution

WHEREAS, the Lewis County Board of County Commissioners (BOCC) has reviewed Consolidated Contract #CLH32057 Amendment #2 between the Washington State Department of Health (DOH) and Lewis County; and

WHEREAS, the Consolidated Contract transfers federal and state funding to Local Health Jurisdictions (LHJs) for planning and local implementation of public health prevention and early intervention programs and activities; and

WHEREAS, the duration of this funding is three years (2025-2027), and it is allocated annually with amendments throughout the year; and

WHEREAS, Amendment #2 increases funding by $43,648 for a revised maximum consideration of $2,945,392; and

WHEREAS, this amendment also (1) amends Statements of Work for the WIC Nutrition Program and (2) adds Statements of Work for the Office of Drinking Water Program-YR2 Contaminated Water System Provision; and

WHEREAS, this amendment also amends Exhibit C Federal Grant Awards Index, incorporated by reference, and located in the ConCon, Funding & BARS library; and

WHEREAS, it appears to be in the best public interest to authorize the execution of said contract.

NOW THEREFORE BE IT RESOLVED that the BOCC hereby approves Consolidated Contract #CLH32057 Amendment #2 to (1) increase funding by $43,648 for a revised maximum consideration of $2,945,392, (2) add Statements of Work for one program, (3) amend Statements of Work for one program, and (4) amend Exhibit C Federal Grant Awards Index; and the BOCC authorizes the Director of Public Health & Social Services to sign the same.
