Summary:

The State Developmental Disabilities Administration contracts with Lewis County to oversee the administration of grant money for services provided to individuals with developmental disabilities. This subcontract provides funding for Morningside to provide individual supported employment, community inclusion and job foundation services to eligible persons who reside in Lewis County.

The purpose of this amendment is to update service requirements, add to the statement of work and increase the budget amount.

Resolution

WHEREAS, the Lewis County Board of County Commissioners (BOCC) has reviewed Amendment #M01-0164-22-A to Contract #M01-0164-22 with Morningside to provide individual supported employment, community inclusion and job foundation services to eligible persons who reside in Lewis County; and

WHEREAS, Amendment #M01-0164-22-A increases funding by $118,200 for a new total maximum consideration of $468,500; and

WHEREAS, this amendment also updates service requirements and adds to the statement of work; and

WHEREAS, the contract duration is July 1, 2022, through June 30, 2023; and

WHEREAS, it appears to be in the best public interest to authorize the execution of said contract.

NOW THEREFORE BE IT RESOLVED that the BOCC hereby approves Amendment #M01-0164-22-A to the contract with Morningside to increase funding by $118,200 for a new total maximum consideration of $468,500, update service requirements, and add to the statement of work of this contract through June 30, 2023; and the BOCC authorizes the Director of Public Health & Social Services or her designee to sign the same.
