Summary:

Approving an addendum to County Professional Services Agreement #R01-0173-22-A between Lewis County Public Health and Social Services (PHSS) and Reliable Enterprises to correct scrivener's errors listing incorrect contract end dates. The contract end date is through June 30, 2023.

Resolution

WHEREAS, the Lewis County Board of County Commissioners (BOCC) approved a professional services agreement between PHSS and Reliable Enterprises to provide case management services to those persons residing in Reliable Enterprises properties within Lewis County via Resolution 22-335 on October 18th, 2022; and

WHEREAS, the professional services agreement contained scrivener’s errors that incorrectly listed contract end dates of June 30, 2022 on the executive summary and June 31, 2022 on the contract ; and

WHEREAS, PHSS and Reliable Enterprises wish to correct the scrivener’s errors through the attached revised contract agreement which corrects the contract end dates on the executive summary and the contract through June 30, 2023 ; and

WHEREAS, it appears to be in the best public interest to authorize execution of said contract.

NOW THEREFORE BE IT RESOLVED that the amended professional services agreement between PHSS and Reliable Enterprises to provide case management services to those persons residing in Reliable Enterprises properties within Lewis County corrects scrivener’s errors for a contract end date through June 30, 2023, is herby approved and the Director of Public Health and Social Services (PHSS) is authorized to sign the same..
