APPROVE AMENDMENT 3 TO PROFESSIONAL SERVICES AGREEMENT "COMMUNITY COVID-19 DRIVE THRU VACCINATION EVENT" BETWEEN LEWIS COUNTY PUBLIC HEALTH AND SOCIAL SERVICES AND PROVIDENCE HEALTH & SERVICES WASHINGTON D/B/A PROVIDENCE MEDICAL GROUP SOUTHWEST WASHINGTON
WHEREAS, the Lewis County Board of County Commissioners (BOCC) has reviewed Amendment 3 to Professional Services Agreement "Community COVID-19 Drive Thru Vaccination Event” with Providence Health & Services Washington d/b/a Providence Medical Group Southwest Washington to provide COVID-19 Vaccination events for community members in non-traditional healthcare settings; and
WHEREAS, these services include vaccine management, patient scheduling, vaccine administration, on-site check-in and a COVID vaccine call and email center; and
WHEREAS, this amendment does not change the original budget consideration; and
WHEREAS, this amendment revises the original contract duration from January 21, 2021, through July 20, 2021, to October 1, 2020, through July 20, 2021, in order to reimburse Providence for services rendered; and
WHEREAS, it appears to be in the best public interest to authorize execution of said amendment.
NOW THEREFORE BE IT RESOLVED that Amendment 3 to Professional Services Agreement "Community COVID-19 Drive Thru Vaccination Event” between Lewis County and Providence Health & Services Washington d/b/a Providence Medical Group Southwest Washington to provide COVID-19 Vaccination events for community members in non-traditional healthcare settings from October 1, 2020, through July 20, 2021, and does not change the original budget consideration, is hereby approved, and the Interim Director of Public Health and Social Services is authorized to sign the same.
