Resolution

WHEREAS, the Board of County Commissioners (BOCC), Lewis County, Washington, has reviewed Amendment, #Birdseyemedical_MassVacCOVID-19-B, with Bird’s Eye Medical; and

WHEREAS, funding in the amount of $280,000 is added for a total amount of $1,030,000 provided for the period of January 21, 2021, through October 18, 2021; and

WHEREAS, funds will be used to provide COVID-19 mass vaccine consultation, coordination, and administration for persons residing in Lewis County; and

WHEREAS, it appears to be in the best public interest to authorize the execution of said contract.

NOW THEREFORE BE IT RESOLVED that Amendment, #Birdseyemedical_MassVacCOVID-19-B,  between Lewis County and Bird’s Eye Medical to provide COVID-19 vaccine consultation, coordination, and administration in the amount of $280,000 for a total of $1,030,000 from January 21, 2021, through October 18, 2021, is hereby approved and the Director of Public Health & Social Services is authorized to sign the same.
