APPROVING AMENDMENT A TO THE MOU BETWEEN VALLEY VIEW HEALTH CENTER AND LEWIS COUNTY TO EXTEND ENDING DATE, PROVIDE TESTING STRIKE TEAMS, AND ADMINISTER COVID-19 VACCINES TO RESIDENTS OF LEWIS COUNTY
WHEREAS, the Board of County Commissioners (BOCC), Lewis County, Washington, has reviewed the change of terms in Amendment A, #141P6226-COVIDVAC, between Lewis County and Valley View Health Center; and
WHEREAS, there is no change of funding attached to this amendment and the original contract remains funded at $300,000; and
WHEREAS, this amendment is effective January 21, 2021, through July 20, 2021; and
WHEREAS, it appears to be in the best public interest to authorize the execution of said contract for Lewis County.
NOW THEREFORE BE IT RESOLVED that Amendment A, #141P6226-COVIDVAC, between Lewis County and Valley View Health Center is extended through July 20, 2021, to provide testing strike teams and administer COVID-19 vaccines to residents of Lewis County is hereby approved and the Director of Public Health & Social Services is authorized to sign the same.
