Resolution
APPROVE AMENDMENT BETWEEN LEWIS COUNTY AND MORNINGSIDE TO ADD ADDITIONAL FUNDS THROUGH DDA IN THE AMOUNT OF $18,990 AND REVISE PLANNED SERVICE RATES
WHEREAS, the Board of County Commissioners (BOCC), Lewis County, Washington, has reviewed Amendment #M01-0090-21-A with Morningside to provide services to individuals with developmental disabilities for Individual Employment and Community Inclusion services; and
WHEREAS, additional funding in the amount of $18,990 for a total maximum consideration of $321,190 through the State DDA is provided for the period of July 1, 2020, through June 30, 2021; and
WHEREAS, planned service rates are revised; and
WHEREAS, it appears to be in the best public interest to authorize the execution of said contract.
NOW THEREFORE BE IT RESOLVED that Amendment #M01-0090-21-A between Lewis County and Morningside to provide services to individuals with developmental disabilities in an additional amount of $18,990 for a total maximum consideration of $321,190 and revise planned service rates from July 1, 2020, through June 30, 2021, is hereby approved and the Director of Public Health & Social Services is authorized to sign the same.
